WHAT IS SHAFFER PLAZA?

Shaffer Plaza consists of three, eleven bed intermediate care facilities that provide

  • Twenty-four hour staffing
  • Daily, off grounds activities which enhance self esteem through socialization and recreation
  • Individualized program planning and provision of services by qualified staff

WHAT PROFESSIONAL SERVICES ARE PROVIDED?

The Jefferson County Board of DD and Shaffer Plaza provide the following professional services to individuals residing at Shaffer Plaza:

  • Occupational Therapy
  • Physical Therapy
  • Speech Therapy
  • Dietician Services
  • Quartly on site physician services
  • Nursing services
  • Follow through on all medical, dental, and eye appointments as needed
  • Behavior Management
  • Socialization/ Recreation Services
  • Annual Service Plan meetings with quarterly review

WHO IS ELGIBLE FOR SHAFER PLAZA RESIDENTIAL SERVICES?

If you have a family member with a developmental disability living at home, he or she may be eligible. The family member with disabilities muat be ELIGIBLE fo County DD services, and should be placed on the waiting list for residential services through the Board's Service Coordination Department.

WHO SHOULD APPLY TO LIVE AT SHAFFER PLAZA?

Any qualifying consumer may apply, Admission is based on:

  • Level of care the individual is determined to have a need for
  • Availability of beds
  • Decision of core admitting team comprised of a broad spectrum of professionals.

WHO PROVIDES THE SERVICE?

Shaffer Plaza is administered through Jefferson County Board of DD. The funds are provided through the direct billing of Medicaid.

HOW MUCH WILL SHAFFER PLAZA SERVICES COST MY FAMILY?

Shaffer Plaza is an intermediate care facility licensed by the Ohio Department of Health and DD. As such, it is a qualified Medicaid facility. An individuals' qualifying resources are generally considered to be:

  • Monthly Social Security Income
  • Any family retirement funds for which the individual is designated beneficiary
  • Possession of, or ability to gain Ohio Medicaid Card
  • Private pay can be arranged

The mission of the Jefferson County Board of Developmental Disabilities if to establish quality supports focused on the individual which will provide opportunities for informed choices and achievements of those choices. These choices will provide an opportunity for individuals to plan their futures in where they live, work, learn, and develop social relationships.
